One dead after two vehicle accident in Vigo County

Vigo County, IND – Just before 1 p.m. Saturday the Vigo County Sheriff’s office was called to a two vehicle crash on Highway 40, just east of State Road 340 in Eastern Vigo County.

Officials say that the driver of a tan Dodge Caravan had fled the scene and the occupants of a red Ford Mustang had to be extricated from the vehicle.

The driver of the Mustang was airlifted to Methodist Hospital in Indianapolis and the passenger was taken to Union Hospital in Terre Haute and was pronounced dead.

The driver of the Mustang was identified as Yesli Perez-Lopez, 25 and the passenger was later identified as Fernanda Martinez, 20, of Brazil, IN.

Shortly after the crash, an Indiana State Conservation officer and a Clay County Sheriff’s Deputy took John Fasig of Martinsville, Illinois into custody in connection with the crash.

Fasig was also airlifted to Methodist Hospital for spine and neck injuries sustained in the crash.

Police are waiting on test results to see if drugs or alcohol may have plaid a factor in the accident.

Police also searched for the passenger of the van, who fled from the scene and found 48 year-old Dawn LaFountain. She was preliminarily charged with leaving the scene of a fatal crash.
